Villa Park residents that are eligible for the reduced cab fare may get a replacement card which will not list personal information. You may bring in your current card so staff can cut out the picture and apply it to the new card. If you prefer a different picture bring a one and a half inch by one and a half inch photo with you to the Village Hall located at 20 S. Ardmore, Villa Park, IL 60181. A new number will be assigned to the new card.

Residents who do not bring in a current card to be replaced will need to bring in the following:

• Identification showing their current address and date of birth.
• A picture one and a half inches by one and a half inches that will be applied to the card.

Residents that are not sixty five or over, will in addition to the two items listed above, need to bring in a letter from their Doctor affirming that they are a person with a disability.

It will take a few days for the Village Clerk to process the card before it will be ready to present to the cab driver.
